2020-2021 FMS School Supply List is HERE!!! 

Required: Face coverings - 2 reusable, washable. Images on coverings should follow the district dress code. According to the Center for Disease Control: cloth face covering should: fit snugly but comfortably against the side of the face; completely cover the nose and mouth; secured with ties or ear loops; includes multiple layers of fabric; allows for breathing without restriction; can be laundered and machine dried without damage or change to shape. Face shields that cover the nose and mouth and can be cleaned are acceptable.

 

 

 

 Athletics:

******NEW IMPORTANT INFORMATION!!!!****** Updated January 7th, 2021....We are now allowing 2 spectators per athlete to attend their students HOME GAME ONLY. This will be monitored and enforced with a check in system prior to the game. There will still be no visiting fans allowed. Parents WILL NOT be allowed to travel to away games to watch. We will be live streaming the games on Facebook. Please contact your students coach for more details on this. 

 
Fremont Boys Basketball will be starting on January 11th.  7th Grade Tryouts will be starting on January 11th at 5:45 AM to 7:20 AM.  8th Grade Tryouts will be starting on January 11th from 3:30 PM to 5:00 PM.  Be sure to have appropriate court shoes as well as shorts, t-shirt, waterbottle, and a mask for tryouts/practices.If you do not have a current Sports Physical or Concession form those will be needed on the 11th when you arrive for tryouts.  You also need to fill out the COVID waiver form if you have participated in a previous sport this year at FMS.  
  
Don't forget Athletes--Make sure that you have a signed COVID waiver, a physical and a concussion form all turned into your coaches. You can find these forms by clicking here!! Fee will be required for the winter seasons.  Exact cash or check can be paid to the FMS office, or you have the ability to pay by card for a convenience fee. The details for that option are below under the heading Online Payments(bolded in blue).

ONLINE PAYMENTS:

To make payments on our Web store, please select Fremont Middle School on Revtrak. You must select Fremont Middle School or your payment will not go to the proper location. There is a 3.5% convenience fee assessed with the online store. If you prefer not to use this system, exact cash or check is always accepted in the FMS office. Spirit gear is not available for purchase online.

New Students register here: After registering online, you will need to provide an original copy of the following:

  1. An original or certified copy of your child's birth certificate
  2. Up-to-date immunization record(s)
  3. Proof of address (such as a phone or utility bill, or rental contract)
  4. Previous school transcript or most recent report card (if any)
  5. Custody paperwork (if applicable)
